Presentación del caso
Motivo de la consulta
La familia de Pablo acude a su pediatra con su hijo, de 2 años y 11 meses de edad, porque están preocupados por aspectos en su desarrollo.
Anamnesis
Los padres explican que el desarrollo de Pablo fue aparentemente normal hasta los 12-13 meses de edad: miraba, reía, gateo a los 9 meses, jugaban a esconderse, etc. A partir de esa edad empezó la preocupación de los padres porque disminuyó el contacto ocular de su hijo, no decía «papá» ni «mamá», se mostraba más serio, dejó de comer algunos alimentos sólidos, etc. Desde hace aproximadamente medio año aletea cuando algo le gusta mucho (puntualmente). Comenzó la bipedestación a los 14 meses. Actualmente no habla (sólo emite sonidos vocálicos o cadenas de sílabas), no tiene adquirido el control de esfínteres, es cariñoso con sus familiares, pero no busca a los otros niños para relacionarse con ellos en la guardería, no señala, va a buscar a los padres para pedir ayuda si él solo no puede (les estira del brazo o levanta sus brazos para que lo cojan), come alimentos triturados (rechaza los alimentos sólidos que antes comía) y todavía mama para dormirse o cuando está enfermo (lo ayuda a relajarse). Los padres refieren que no reacciona al dolor cuando se cae y que se altera con los chillidos y lloros de otros niños. Si los padres no lo interrumpen, puede pasarse mucho tiempo realizando la misma acción (es muy repetitivo). Juega muy poco de manera funcional, y utiliza los juguetes para golpearlos continuadamente contra el suelo o la mesa. Le gustan los objetos redondos y reacciona con intensas rabietas si se los quitan de las manos. Los padres refieren que cuando le cambian de rutinas reacciona también con protesta o rabietas.
El curso pasado empezó P-2 en una escuela ordinaria, con una adaptación lenta y altibajos a lo largo del curso. Sólo quería ir al patio y allí corría arriba y abajo.
Es hijo único, y los padres inicialmente no refieren ningún antecedente de importancia en la familia, aunque comentan que el abuelo paterno tiene un carácter «especial», nunca ha tenido amigos, es inflexible con las otras personas y no soporta el bullicio y el ruido. Una prima de la madre explicaba que había presentado un retraso en el lenguaje (recuerda que comenzó a hablar bien después de los 5 años), y la madre ha escuchado que a otro primo materno le han diagnosticado un trastorno por déficit de atención e hiperactividad (TDAH) y toma medicación.
Pablo no ha tenido antecedentes médicos de relevancia.

Exploraciones de cribado realizadas por el pediatra
El pediatra sospecha la presencia de alteraciones en el desarrollo sociocomunicativo y posible sintomatología de un trastorno del espectro autista (TEA). Utiliza instrumentos de cribado, como el Cuestionario del Bebé y Niño Pequeño (CSBS-DP), para evaluar competencias sociocomunicativas, y el Cuestionario de Verificación para el Autismo en Niños Pequeños (M-CHAT) como instrumento de cribado de TEA, y realiza exploraciones médicas complementarias.
CSBS-DP
La escala CSBS-DP valora 7 predictores del lenguaje: emoción y uso del contacto ocular, uso de la comunicación, uso de gestos, uso de sonidos, uso de palabras, comprensión y uso de objetos en el juego. Permite evaluar las competencias sociocomunicativas entre los 6 meses y los 2 años y medio de edad en niños, y entre los 6 meses y los 6 años de edad cronológica, así como detectar una posible sintomatología correspondiente al TEA y otros problemas del desarrollo sociocomunicativo.
En el caso de Pablo, la puntuación en la escala CSBS-DP resulta preocupante en todas las áreas de comunicación, lenguaje, simbolización y total, tal como se refleja en la tabla siguiente:
M-CHAT
El M-CHAT, en su versión revisada y con seguimiento (M-CHAT-R/F), es un cuestionario combinado con preguntas de seguimiento para detectar alteraciones del desarrollo dentro del TEA, válido para niños de entre 16 y 30 meses de edad, que consta de 2 etapas. En un primer estadio, los padres deben completar un total de 20 preguntas dicotómicas (sí/no). Las preguntas 2, 5 y 12 «sí» indican riesgo, y las otras preguntas «no» indican riesgo. En una segunda fase, el profesional pregunta sobre los ítems del cuestionario que el niño ha fallado. Se suman con una puntuación total y se categoriza el M-CHAT-R. Se considera un resultado positivo si la puntuación total es de ≥3, o da positivo para más de 2 ítems considerados críticos. Una puntuación del M-CHAT-R entre 8 y 20 se considera de alto riesgo, por lo que se puede prescindir de las preguntas de seguimiento y derivar al paciente directamente al especialista. Si la puntuación del M-CHAT-R es <3, se considera de bajo riesgo y el pediatra se encargará de supervisar el seguimiento del paciente. Una puntuación de entre 3 y 7 se considera de riesgo medio y se combina con las preguntas de seguimiento. Si el resultado del M-CHAT-R/F sigue siendo ≥2, se considera que el cribado es positivo y hay que derivar al paciente al equipo especializado en neurodesarrollo para su evaluación y tratamiento.
En el caso de Pablo, ha fallado 13 ítems, de los cuales 5 son críticos, por lo que se aconseja derivarlo directamente a especialistas en el desarrollo para su evaluación y tratamiento.
Pruebas médicas complementarias
Se considera que, por la historia clínica del niño, los resultados del cribado CSBS-DP, con preocupación en todas las áreas, y el alto número de fallos en los ítems del M-CHAT-R, hay que derivarlo al equipo multidisciplinario especializado en neurodesarrollo de la zona para su evaluación y tratamiento.
Exploración psicológica-evolutiva-médica por parte del equipo multidisciplinario local
El equipo multidisciplinario especializado en neurodesarrollo de la zona realizó la exploración del menor, en la que le sometieron a las siguientes pruebas:
Inventario de desarrollo de Battelle
El inventario de desarrollo de Battelle es un instrumento de evaluación que permite apreciar las habilidades en el desarrollo del niño y su progreso en 5 áreas diferentes:
Además, proporciona información sobre los puntos fuertes y débiles en estas áreas del desarrollo con el fin de facilitar la elaboración de programas de intervención individualizados.
La puntuación global (centil 1; puntuación típica [T]= 27) está por debajo del nivel esperado en todas las áreas del desarrollo.
Escala adaptativa de Vineland
La entrevista estructurada de Vineland se administra para establecer el nivel adaptativo de Pablo en las áreas de comunicación, hábitos de autonomía, habilidades sociales y habilidades motoras. La escala proporciona un esquema del desarrollo normal que puede usarse repetidamente para la medición o la valoración de los cambios durante el crecimiento o el tratamiento del niño, además de una pauta para comprobar hasta qué punto el sujeto está limitado en su desarrollo social por restricciones ambientales, falta de oportunidad ambiental, sobreprotección de los padres u otras circunstancias. Los ítems de la escala están ordenados por dificultad creciente y representan una maduración progresiva. A continuación se muestran los resultados obtenidos:
Los resultados de Pablo dan una suma de las áreas (puntuación estandarizada) de 54.
Respecto a la conducta adaptativa compuesta, la puntuación estándar fue ≤20 (percentil <0,1) y el nivel de adaptación equivalente, bajo.
Cuestionarios Achenbach System of Empirically Based Assessment (ASEBA) para padres (CBCL) y profesores (TRF 1.5-5)
El CBCL/TRF 1.5-5 (Child Behavior Checklist) es una escala que ofrece unos resultados englobados en 7 dimensiones (emocionalidad reactiva, quejas somáticas, ansiedad y depresión, aislamiento, problemas de sueño, inatención y conducta agresiva), que a la vez se engloban en 2 supradimensiones (internalización y externalización de problemas), así como una puntuación total.
Los resultados de Pablo en el cuestionario de cribado para problemas de alteraciones en el desarrollo en diferentes áreas tuvieron una puntuación significativa en «aislamiento» en el CBCL-padres, y también en «emocionalmente reactivo», «aislamiento» e «internalización de problemas» en el TRF completado por los profesionales de la guardería (kindergarden).
Escala de Comportamiento de las Funciones Ejecutivas (BRIEF-P)
La versión infantil de la BRIEF-P permite evaluar los aspectos más cotidianos, conductuales y observables de las funciones ejecutivas en niños de 2 a 5 años, tanto en el entorno escolar como en el familiar. Consta de 5 escalas clínicas (inhibición, flexibilidad, control emocional, memoria de trabajo, planificación/organización) agrupadas en 3 índices (índice de autocontrol inhibitorio, índice de flexibilidad e índice de metacognición emergente) que, a la vez, se agrupan en una escala o índice global (IGE). Las puntuaciones típicas tienen una media de 50 y una desviación estándar de 10. Por tanto, consideraremos significativas unas puntuaciones ≥65.
BRIEF-Escuela
En el caso de Pablo, la BRIEF completada por los profesionales de la guardería, excepto en el área de inhibición, que da una puntuación límite, resulta alterada en todas las áreas de la función ejecutiva.
Entrevista diagnóstica ADI-R
La ADI-R es una entrevista semiestructurada que se efectúa al progenitor y que permite obtener una evaluación profunda en la que se exploran las 3 grandes áreas nucleares de dificultad del TEA (lenguaje/comunicación, interacciones sociales recíprocas y conductas e intereses restringidos, repetitivos y estereotipados) definidas en el DSM-5. La información recogida se codifica y se traslada a unos sencillos y útiles algoritmos que orientan el diagnóstico y la evaluación de la situación actual del sujeto.
En el caso de Pablo, los resultados de la ADI-R indican una alteración significativa en las 4 subescalas del algoritmo, que muestra una alta sensibilidad y especificidad para el diagnóstico clínico de TEA.
Escala de Observación para el Diagnóstico del Autismo-2 (ADOS-2)
La ADOS-2 es un instrumento de observación semiestructurado que incluye diversas actividades que el entrevistador va presentando al sujeto con la finalidad de valorar aspectos de su comunicación, lenguaje, interacción social recíproca, comprensión socioemocional, imaginación, y presencia de intereses y/o conductas repetitivas o estereotipadas u otras conductas anómalas. Es válido para niños mayores de 12 meses y todas las etapas posteriores, incluida la edad adulta. La prueba consta de 5 módulos que se ajustan a la capacidad de lenguaje y edad cronológica del sujeto. Se utilizan algoritmos diagnósticos simples de completar, con puntos de cohorte para autismo o espectro autista.
Las puntuaciones pueden ser: 0 (suele expresar que no hay alteración), 1 (ligera alteración que no es tan interferente y anómala como la puntuación 2) o 2 (alteración clara del comportamiento que se valora). A Pablo se le administró el módulo 1.
Puntuación total global (AS + CRR)= 20 (6/10). Puntuación total global mayor o igual que el punto de corte de autismo (pocas palabras o ninguna: total global >16).
La puntuación en el algoritmo del ADOS-2, módulo 1, de Pablo está por encima del punto de cohorte para el autismo, por lo que muestra una alta sensibilidad y especificidad para el TEA.
Exploraciones genéticas
Se complementaron las exploraciones médicas realizadas por el pediatra con un estudio genético, que incluía un microarray genético y un estudio del frágil X, con resultados dentro de la normalidad.
Conclusiones diagnósticas
Se concluye que, por la historia de su desarrollo, la exploración realizada y las observaciones durante la entrevista, Pablo presenta una sintomatología dentro de los TEA (299.00), según los criterios diagnósticos del DSM-5:
